Important Instruction for MS University Baroda CCC Exam Online Registration :
1 .On clicking “Proceed for Registration”, you will find a new screen with 3 options available as shown below. For first time registration click on “New Registration” button as shown below.
2.On clicking above button a form will be displayed as shown below.
3.Please fill the Name (according to the Service Book), Birth Date, Mobile number, E-mail Address and other details available in the registration form properly. Please check the filled details before submitting the form. After submitting the form, your need to note the
Application Number provided to him. Later on this Application Number will be used for any correspondence related to CCC Exam.
4. After submitting the form, you will get an email in your E-mail account. It will contain a Registration Number. Please note that number. Please keep it confidential and don’t share it with others. It will be useful in modifying details in the registration form up to 3 days
after the date of Registration.
5.Please make sure that proper Exam type have been selected while filling the details and proper Exam fees has been paid online using Debit card, Credit card or Net banking. Fee details are as below.
For Theory and Practical both Exams: Rs. 200
For Theory Exam: Rs. 100
For Practical Exam: Rs. 100
6.To modify any detail in the form, Enter the Registration number and then click on “Submit” button as shown below.
7.On clicking above button it will allow you to modify the details. Make necessary changes and click on “Update” button to save the changes made in the form.
8. After submitting the Registration form takes print out of the same. To take print out EnterApplication Number and click on “Print” button as shown below.
9.After clicking on above button it will display a form that contains your details as shown below. Take print out of the whole page.
10.10. In this printed registration form, you need to sign and get signature, stamp of the Head of Department/Principal/reporting authority. Without the signature of candidate and sign, stamp of the head, form will not be accepted.
11. If photo is not visible properly in the printed registration form then affix latest passport size photographs in the space provided and also send one photograph along with the form.
12. Name written in registration form and name in your Identity card given by concerned office must be same. If we find any difference in both names, your form will be rejected.
13. Your require to submit following documents along with, a. Original Registration form Printout with proper attestation of candidate and HOD with Stamp.
b. Copy of any one Photo ID proof (Current Organization’s ID-Card preferable)
c. Copy of Appointment Order / Letter
d. Please write “CCC Exam Form” on top of the Envelope.
14. Candidate must submit his/her application by Speed Post / by courier / by hand delivery
at the following address without fail within 15 days from date of registration.

Passport / Election Card / PAN Card (ANY ONE) without fail.
